Flu vaccinations are designed to provide protection against the seasonal influenza virus.
A flu shot clinic will be held on
Wednesday, October 21, 2026 from 11 AM to 4 PM
in the lobby of the Performing Arts Center at Sioux Valley Schools
As your local healthcare provider, we are able to provide the influenza vaccine with the following information:
1) Consent form ~ VACCINE CANNOT BE GIVEN WITHOUT THIS FORM. It must be signed by a parent or legal guardian. The student will sign only if age 18 or older.
2) If we are submitting information to health insurance, we will need the health insurance information form and a copy of your insurance card. Please return a copy of both sides of your insurance card.
3) If not submitting to insurance, please return consent form only with $30 payment (checks can be made to Volga Medical Clinic).
**It is required that a parent or guardian be present with all elementary students for the flu shot clinic.
