Greetings,
Parents- please sign up for a time slot (Sign Up Genius link is below) for your son/daughter for 2022-23 Baseline Concussion Testing and please complete and bring along a consent form. The consent form is attached to this email and a link is also provided to sign up. Check in will just outside the library and testing will be in the HS Study Hall Room.

*This is required for all student athletes grade 7th-12th participating in the 2022-23 school year. (6th graders in MS Cross Country or MS Wrestling should take it as well)
Testing Days and Times are:
(Mon) July 25th 9:00AM – 3:00PM
(Tues) July 26th 9:00AM – 3:00PM
If you need to take it another time, we will coordinate that with the trainer.
